Baldwin Bone & Joint is committed to fulfilling your requests for medical records in a safe, secure and timely manner. To better serve you, Baldwin Bone & Joint PC has partnered with Sharecare Health Data Services to handle distribution of patient medical records.
To initiate your request, please complete each section of the Authorization for Disclosure of Health Information form.

In most cases, records requests are fulfilled within 24 – 48 hours of receiving the signed authorization form.

Medical records can now be delivered to patients in a secure, encrypted digital format via secure email. Electronic delivery of medical records results in faster turnaround time.

Most records requests will incur a fee and those charges are typically less than $20. Requests for fifteen pages or less are complimentary.
